I, _____________________________________ am paying a course fee to take the Utah or Florida or both pistol permit class(es) to legally carry a concealed firearm outside of NY State. I understand that while carrying concealed, I must obey the local, state, and federal laws applying to the place in which I carry.

For example, I am a New York resident with a Utah permit carrying in Toledo, Ohio. The laws of Toledo, Ohio will apply to me.

I acknowledge and understand that:

(a) this course covers excerpts of Utah/Florida State and US Federal laws relating to concealed carry with a Utah/Florida Permit. New York State has it’s own separate laws not covered in this course;

(b) material in this course DO NOT apply in New York State. The Utah/Florida Permit has NO RELEVANCE to purchasing, possessing, carrying, using or owning a firearm in New York State.

I recognize that this course does not guarantee that I will receive any permit from the State of Utah/Florida, and that permits are granted based on properly completed, submitted, and approved applications.

I also acknowledge and agree to the following:

(a) I must deal directly with Utah and/or Florida’s government issuing agencies to secure a CCW permit;

(b) no other State’s pistol permit course material has been mixed in with the materials in this course;

(d) course fees are completely non-refundable;

(e) the instructor is not suggesting that I carry a concealed firearm, use a firearm in self-defense, or even own a firearm;

(f) the ability to carry concealed across the USA with a permit is based on federal, state and local laws that can change periodically;

(g) the instructor has not given any legal advice to me either verbally or through course materials on anything to do with concealed carry;

(h) I am solely responsible to check all applicable laws before purchasing, transporting, storing, carrying or even thinking about using a concealed firearm in any way;

(i) I will stay up to date on any changes in the laws and permit programs as they apply to me;

(j) I will take every opportunity to ask questions about the course and this release, as needed;

(k) The instructor(s) and their companies have no liability for my actions or conduct with a firearm whatsoever and shall not be subject to legal action by me, my family, my heirs, my assigns, or anyone else.
